I, ("Participant"), in consideration of being permitted to participate in any and all programs, activities, or events conducted or sponsored by Association of Seniors of Indian Origin ("ASIO"), including but not limited to cultural, educational, recreational, social, and physical activities, for myself and, where applicable, on behalf of my minor children, knowingly and voluntarily assume all risks associated with such participation, whether known or unknown.
To the fullest extent permitted by law, I hereby irrevocably waive, release, and forever discharge ASIO, its officers, directors, employees, agents, volunteers, members, the venue owner and operator, and all other persons or entities involved in the planning, organization, or execution of the programs ("Released Parties") from any and all claims, demands, actions, liabilities, damages, losses, costs, or expenses of any kind whatsoever, arising out of or related in any way to my participation, including, without limitation, claims arising from the negligence of any of the Released Parties.
I further agree that I will not institute any lawsuit or claim against any of the Released Parties for any matter covered by this Release.
I acknowledge that I have read and fully understand this Release and voluntarily agree to be bound by its terms. If any portion of this Release is held to be invalid or unenforceable, the remaining portions shall remain in full force and effect. This Release shall be governed by the laws of the State of Oregon.
